Use this template to make laminated wallet cards for all families, staff, and other adults in your program. Give them new cards whenever information changes.
If you create your own emergency wallet card, include this information:
- Program address
- Program primary phone
- Program alternate phone
- Off-site relocation point 1 address
- Off-site relocation point 2 address
- Program emergency communication methods (e.g., website, social media, communication apps)
- Note about a photo ID being needed for all emergency contacts who pick up children
