Complete this form and share it with all community partners who take part in your emergency preparedness planning and evacuation procedures.

If you create your own form to share with community partners, include this information:

  • Name of program
  • Location
  • Name and contact information of your:
    • Program director
    • Incident leader
    • Communication lead
  • Building occupancy (number of adults, number of children)
  • Age groups of children in building (e.g., infants, toddlers, preschoolers)
  • Attached floorplan of your facility
  • Shelter-in-place locations
  • Names and addresses of relocation sites
  • Special considerations for moving adults and children with disabilities and special health care needs, if applicable
